Following CADDA’s recent FemTech Innovators Event at UCL, one message came through loud and clear: innovation in women’s health is both timely and essential. Women’s health is health, and there remains a significant need for further research, investment, and innovation to ensure women’s needs are adequately represented and that effective diagnostic solutions are developed and adopted.
While the discussions focused on FemTech, many of the challenges raised are not unique to women’s health. In fact, they reflect broader themes seen across the UK innovation landscape. Whether developing diagnostics, digital health solutions, medical devices, or therapeutics, successful innovators tend to navigate three interconnected pillars: evidence, commercialisation, and real-world implementation.

These pillars are not sequential or mutually exclusive. The most successful innovations are designed with purpose from the outset, developed with a clear understanding of user needs, and shaped around the realities of the healthcare systems they aim to serve.

Pillar 1: Evidence

Generating robust evidence remains one of the most critical and often most challenging parts of the innovation journey. For healthcare innovators, evidence generation extends far beyond clinical validation. It encompasses intellectual property considerations, legal and regulatory requirements, stakeholder relationships, and the practicalities of engaging with end users. Accessing the right patient populations and clinical partners can be straightforward for some innovations, while for others it can be significantly more complex due to ethical, legal, or operational constraints.

One recurring theme from the event was that evidence generation is fundamentally a partnership activity. Success often depends on building relationships, maintaining visibility within the ecosystem, and actively participating in relevant networks and events. Funding opportunities such as Innovate UK Catalyst programmes can play an important role in supporting evidence generation, but they often require substantial preparation, collaboration, and resource commitment. Identifying shared objectives between innovators, clinicians, patients, healthcare providers, and industry partners can significantly accelerate progress and create stronger foundations for adoption.

Pillar 2: Commercialisation

Generating evidence is only the beginning. The next challenge is translating that evidence into a sustainable commercial pathway. Within women’s health, as in many healthcare sectors, there are multiple routes to market. Each pathway raises important questions: Who is the customer? Who pays for the product? Who ultimately benefits? The answers can vary considerably depending on the innovation and the healthcare setting.

A particular challenge for those organisations who operate internationally, is the acceptance of evidence generated abroad. Innovators often face difficulties when attempting to transfer data and clinical evidence from overseas studies into NHS and regulatory decision-making processes. The differences between UK and US healthcare markets were also highlighted. In the United States, insurers, employers, pharmaceutical companies, and healthcare providers often play a more direct role in adoption decisions. In contrast, while NHS adoption remains a key objective for many UK innovators, the role of private healthcare providers continues to grow and may offer additional routes to scale.

The fragmented nature of the NHS can add further complexity. Different regions, trusts, and service providers may have varying priorities, procurement processes, and operational requirements. This fragmentation is often most apparent when moving from a successful pilot project to a funded procurement opportunity. Many innovators encounter difficulties demonstrating sustainability beyond the pilot phase. Establishing appropriate pilot structures, defining meaningful KPIs, and demonstrating long-term value remain critical factors in securing broader adoption.

Investor sentiment was another topic of discussion. Several innovators noted that investor enthusiasm can change when the primary route to market involves NHS adoption, reflecting perceptions around procurement timelines and market complexity. As a result, many companies are pursuing multiple commercial pathways simultaneously, including international markets and private healthcare opportunities.

Pillar 3: Real-World Implementation

Perhaps the most important challenge after the evidence has been collected and investments have been secured is the adoption in real-world healthcare settings.

A common question is how an innovation that has been successfully implemented in one NHS trust can be replicated across five, or even ten other Trusts. The reality is that healthcare organisations frequently operate differently. They have distinct pathways, priorities, workforce structures, and operational requirements. As a result, an approach that succeeds in one organisation may require adaptation elsewhere.

While there is no single solution, one promising approach is to focus on system enablers. Innovators who clearly demonstrate how their solution helps healthcare organisations achieve existing objectives are often better positioned for adoption than those who focus solely on the technology itself.

Interestingly, discussions also suggested that healthcare organisations are not always seeking the most academically rigorous solution. While robust evidence remains essential, decision-makers are often equally interested in practical, proven solutions that fit within existing workflows and can be implemented without creating additional burden. For diagnostics innovators in particular, understanding the environment in which a solution will be deployed is critical. Successful adoption requires a deep appreciation of the systems, processes, and people involved.

Finally, communication remains one of the most overlooked factors in innovation success. Innovators must be able to articulate the value they bring in language that resonates across the entire healthcare system not only with clinicians, but also with commissioners, managers, procurement teams, investors, and policymakers. Value extends beyond cost savings alone, it includes improved outcomes, increased efficiency, enhanced patient experience, and support for wider system objectives.
The ability to communicate that value effectively, whether to healthcare leaders or investors, can often be as important as the innovation itself.

As the FemTech sector continues to mature, there is much it can learn from the broader healthcare innovation ecosystem. Equally, there is much the wider sector can learn from the urgency, creativity, and patient-centred focus that is driving innovation in women’s health today.
